If you meet the qualifying criteria, your doctor will issue a waiver form and a certification form for medical cannabis

Register with the State

Once your evaluation is complete in Georgia, your doctor will issue a waiver form and a certification form for medical cannabis and submit these forms directly to the Georgia Department of Health.
